A paperless intake pipeline for a fitness business

Design Engineering

Automates waiver + PAR‑Q intake with signature capture, PDF generation, and Notion-backed storage + clearance flagging

Taking a step back

The studio trains athletes at multiple levels, so intake needs to be fast and trustworthy. I mapped a standard waiver + PAR‑Q+ flow into a digital experience that enforces compliance without adding complexity.

Insight –> A step‑based digital flow with conditional logic reduces errors and saves real time for both staff and clients.

I leaned into a fast, creative build loop in Cursor —sketching the flow, wiring validation, and shipping a working intake system in a couple hours. The goal wasn’t just “get a link on the site,” but to turn a frustrating paper process into a real productAdmin login with role-based permissions so team members could upload and organize visual content securely.

Bringing a product to life

He asked for a simple waiver link and a message on his site. I took it further: in a couple hours of vibe‑coding in Cursor, I built a full intake system that works directly with the Notion integrations I’d already pitched for his organization.

Features

  1. Multi‑step waiver + PAR‑Q+ flow

  2. Canvas‑based signatures

  3. Minor detection + guardian signature requirements

  4. Conditional PAR‑Q+ fields + “Needs Clearance” flagging

  5. Signed PDF generation and storage

  6. Notion database entry creation

  7. Anti‑spam honeypot and rate limiting

AJ's reaction

  • Immediate time savings by removing printing + manual filing

  • Less client friction with a single digital link to complete intake

  • Clearer review for any PAR‑Q+ “Yes” responses

→ I think he's happy.

Learnings

  • Even a small workflow change can unlock huge time savings for operators.

  • Conditional logic is essential for medical intake—fast for most users, detailed for the few who need it.

  • Integrating with the client’s existing tools (Notion) makes adoption friction‑free.

→ Personal software that makes a businesses life easier.
